Immediately after completing the 8 section online
application please log-in*
and submit the required forms. Your application will
not be considered until all of the forms have been
submitted. The required forms, which can be downloaded
after you log-in to your account, are: |

· Medical Emergency Form |
· Copy of Passport |
· Date Request Form |
· High School Transcripts for Israeli
Citizens. |
| · Army exemption paper for Israeli citizens and children
of an Israeli citizen. |
· Schedule Phone Interview (you will be
contacted by our office letting you know when
to call in) |

*After completing the 8 section online application
you will receive an email from MAYANOT with a username
and password so you can log-in to www.mayanotisrael.com and
submit the medical form, passport copy and other required
information.. |

To avoid being placed on a waiting list, all forms
and the deposit should
be submitted immediately after completing the online
application. |

Applicants will not be considered
for a trip until the online application, a deposit
and all required forms have been submitted. |
