Forms

Immediately after completing the 8 section online application please log-in* and submit the required forms. Your application will not be considered until all of the forms have been submitted. The required forms, which can be downloaded after you log-in to your account, are:

• Medical Emergency Form
• Copy of Passport
• Date Request Form
• High School Transcripts for Israeli Citizens.
• Army exemption paper for Israeli citizens and children of an Israeli citizen.
• Schedule Phone Interview (you will be contacted by our office letting you know when to call in)

*After completing the 8 section online application you will receive an email from MAYANOT with a username and password so you can log-in to your personal Mayanot account and submit your Date Request form, Application photos, medical form, passport copy and other required information. Please be sure to add info@mayanotisrael.com to your email address book or white list to ensure that you get this email. If you do not receive this email within a week please let us know at info@mayanotisrael.com.

To help avoid being placed on a waiting list, all forms and the deposit should be submitted immediately after receiving your login information .

Applicants will not be considered for a trip until the online application, deposit and all required forms have been submitted.
